We are required by law to protect the privacy of your health information, to give you this Notice explaining our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We take that seriously. The information we hold about you - your name, your contact details, your doctor's order, what we fitted you with and when - is yours, and we treat it that way.
TREATMENT. We use your information to provide equipment and service to you. For example, we may contact your prescribing physician to confirm or clarify an order, or share fitting information with another provider involved in your care.
PAYMENT. We use your information to bill and be paid. For example, we may send information to your insurance company, to a workers' compensation carrier, or to a billing company acting on our behalf, to confirm coverage and obtain payment.
HEALTH CARE OPERATIONS. We use your information to run our business well - to review the quality of what we do, to train staff, to handle complaints, and to conduct required audits and accreditation reviews.
BUSINESS ASSOCIATES. Some services are performed for us by outside companies, such as billing or secure record storage. Each one signs a written agreement requiring them to protect your information the same way we do.
AS REQUIRED OR PERMITTED BY LAW. We may use or share your information without your permission when the law requires or allows it, including: public health activities and reporting product problems to the U.S. Food and Drug Administration; reporting suspected abuse, neglect or domestic violence; health oversight activities such as audits, inspections and licensure; responding to a court order, subpoena or lawful request from law enforcement; workers' compensation claims; coroners, medical examiners and funeral directors; organ and tissue donation; approved research; specialized government functions including military and national security; and to prevent a serious and imminent threat to the health or safety of you or another person.
APPOINTMENT AND SERVICE REMINDERS. We may contact you about your equipment, a follow-up, or a scheduled fitting.
PEOPLE INVOLVED IN YOUR CARE. Unless you object, we may share information relevant to your care with a family member, caregiver, or other person you identify.
We will not use or share your information for marketing, sell your information, or share psychotherapy notes without your written authorization. Other uses and disclosures not described in this Notice will be made only with your written authorization. You may revoke an authorization in writing at any time, and we will stop, except where we have already acted in reliance on it.
We are required by law to maintain the privacy of your health information, to give you notice of our legal duties and privacy practices, to abide by the terms of the Notice currently in effect, and to notify you following a breach of your unsecured health information.
We may change this Notice and make the new terms apply to all information we hold. If we change it, we will post the revised Notice in our office and on our website, and a copy will be available on request.
If you believe your privacy rights have been violated, please tell us first - call our Privacy Officer on 470.945.4558. You may also file a written complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, or at hhs.gov/ocr/privacy/hipaa/complaints. WE WILL NOT RETALIATE AGAINST YOU IN ANY WAY FOR FILING A COMPLAINT. Your service will not be affected.
